Age-Appropriate Transition Assessment Planning Guide

Key Takeaways

Ensure we are following a process where the student’s voice and all team members have a chance to discuss and brainstorm adult life goals for the youth.

Take time to develop goals and then discuss the different skills, education, and experiences needed to help a student reach these goals. That is the foundation of transition assessment - helping students build the skills to achieve their goals.

Remember, exploration is a key component of career development. Oftentimes, we only know about those careers and jobs we have been exposed to through our personal experiences (family members, what we’ve seen on TV, etc.). If necessary, early adult life goals can focus on intentionally exploring a variety of career options so the youth can make an informed decision on their adult life goals.

The student’s adult life goals may be vague or undefined at the time of initial planning. Goals may change drastically from new interests or experiences or become more specific and clear. Ongoing AATA will help to refine or change the goals as it is conducted.
