The transition assessment questions generated through discussion from Question Four consists of Part One: Discussion about tools and methods to use for transition assessment and Part Two: Development of a transition assessment plan. Other discussions the team has had along the way may have generated assessment questions too. These questions afford everyone who is a part of the youth’s multi-agency team, including the youth and family, to take an active role in gathering transition assessment data and information. The diversity of perspectives that emerges when everyone partakes in the assessment process results in comprehensive assessment data for the team to unpack and analyze as to how it contributes to informing the youth’s plan for transition to adult life.
