Tools for Developing an Assessment Plan

A tool to be used when documenting a student’s work experience. It can help job coaches focus their notes on certain skills and allow team members from different agencies or departments to capture similar information across settings.

Career Discovery Tools

There are several tools in the Career Discovery Toolkit to help document transition assessment. Use the Employment Observation Report (p. 26), Social Situational Observation Form (p. 28), Vocational Situational Observation Form (p. 30), and other forms and tools to document transition assessment data for the youth. Finding the right form will help team members capture consistent information across trials and settings, but also have clear and concise notes to share with the team members.

What is the plan the team will follow?

The plan identifies the team member(s) that will gather each piece of information, a timeline for collecting data and a date for the team to meet, discuss, compile and draw some conclusions from the additional information. Use this planning form to identify all of the details of the transition assessment plan.
