The process ‘begins with the end in mind’. Adult Life Goals are the very beginning of planning for transition assessment. Once we know the youth’s goals, we can choose individualized assessments to give us information about the student and the skills and education needed for their adult life goal. This will lead us to find additional training, services, or experiences they may need to improve those skills. So, begin with the end in mind by recording what you know about ‘where the youth is going’… even if it is only a vague idea currently.
