Collective Team Knowledge Viewpoint


To ensure we are considering the youth’s goals in various settings and with all of our multi-agency partners and team members, consider asking team members to share or review their service plans, treatment plans, goals, and other plans that team members may have with the student. There may be some alignment with achieving these goals on the way to the student’s adult life goals and vision. Or it may help the team identify goals for the student’s IEP. Remember, part of the benefit of working as a multi-agency team is having all the partners at the table for discussions and planning. This includes sharing data and also planning and goal documents.

Potential Team Members

It is important to include all of our Potential Team Members in our planning meetings when transition and transition services are discussed. These team members may have additional information to add, services to discuss, or even past data that could be updated as part of a new plan.
