Person-Centered Planning


Person Centered Planning is an ongoing problem-solving process used to help people with disabilities plan for their future.

It is critical for the youth and their voice to drive our planning meetings. We must ensure they have set their goals for the future. However, families and other team members can play a role in helping youth identify goals that may align with their preferences, interests, and strengths. The youth may have expressed goals to a team member and forgotten it during their own visioning process. Team members may be able to ask questions about a youth’s past experience that they really enjoyed to see if it is something they would like to continue pursuing. For example, previously volunteering with animals where an educator may have noticed the youth really enjoying it and having a very delicate disposition when caring for the animals.
