NEW: Age-Appropriate Transition Assessment (AATA) Planning Guide for Multi-Agency Professionals

Our new online AATA Tool will introduce you to the planning process for transition assessment. It is important our multi-agency teams intentionally plan for transition assessment to ensure we reduce duplication, build on a student’s progress, and identify individualized assessment to build the youth’s profile in relationship to those skills needed for their adult life goals.   • Learning Aid Ohio, Receive 1-on-1 Tutoring Services for Your Youth

    Connect students learning on IEPs or 504 Plans with experienced learning providers to provide in-person, supplemental support grant project funded till 2024.

  • Using Backwards Planning in the Transition Planning Process

    Planning for a future adult life is critical. Using Backwards Planning as the foundation for that process can enhance the success of the plan. In both Backwards Thinking and Backwards Planning, you begin with the vision or goals for the future adult life as well as the skills and knowledge that will need to be in place to be able to achieve those goals and work through “The Gap.”

  • Autism and How Driving Has Made a Difference: This session highlights stories and experiences from two young men about how driving has made a difference in their lives. They will share the role driving has played, how using transportation is important to quality of life, and how it has opened possibilities for their futures.

  • Students With Complex Support Needs: How to Engage for Focused Planning and Post-High School Success: In this interactive panel, participants will be provided with information regarding various effective practice resources, programs, and strategies to support students and youth with complex support needs to successfully engage in post-secondary education programs that lead to competitive, integrated employment and community engagement. During this session, presenters from NTACT:C will provide examples from multiple universities and states that utilize a cross-systems, person-driven approach to program development, implementation, and student engagement.

  • The Power of Collaboration: Building Effective Multi-Agency Teams: Multi-Agency Planning (MAP) is an effort in Ohio to bring together various agencies that provide services to transition age youth. Organized by county or community, the representatives came together to form local teams to identify and tackle barriers that hinder youth with disabilities. The purpose of the MAP teams is to build capacity for serving youth across agencies, resulting in seamless transition to desired and meaningful adult life outcomes. In this panel session, representatives from five teams throughout the state will discuss the benefits of creating and participating in a MAP team.

Family Engagement: Building a Strong Foundation

In this session, Thomas Capretta from the Ohio Statewide Family Engagement Center will guide participants through identifying the conditions that impact a family’s engagement in their child’s education and what we can do to maximize the way families are engaging and strategies to encourage additional forms of engagement. Participants will have an opportunity to work on applying this knowledge to real scenarios from their education programs, while receiving feedback and collaborating with others. Register now.
