Age-Appropriate Transition Assessment Planning Guide

The Youth Profile is continually updated, summarizing new information and keeping it current and useful for transition planning. Information can be incorporated into official agency documents such as:

IEP (Individualized Education Program) or 504 Plan - The student’s school is responsible for writing and updating the student’s IEP or 504 Plan. The IEP outlines the educational components of a student’s supports including annual goals, specially designed instruction, related services like Occupational Therapy, Physical Therapy and Speech Therapy, and much more.,

ISP (Individual Service Plan) - The local County Board of Developmental Disabilities writes the student’s ISP with input from the student and their team. The ISP outlines services and supports the student utilizes as well as goals related to employment and skill development. The student’s assigned Service and Support Administrator (SSA) writes and updates the ISP.

IPE (Individual Plan for Employment) - Opportunities for Ohioans with Disabilities, or commonly referred to as OOD, is responsible for a student’s IPE. OOD counselors help students identify a goal for a community job that pays at least minimum wage. OOD is able to offer services like career exploration, job coaching, benefits analysis, and more to help the student and team understand their employment goals and how it may impact other aspects of their life.
