Multi-Agency Team Roster
The purpose for the team going through an extensive multi-agency team building process is to ensure we are gathering all the transition data about the student from a variety of sources and perspectives. We know students can have different expectations and experiences within other settings or with other support staff. Gathering information from the family, the youth, and professionals who see the youth in settings other than the school is critical to identifying the skills and supports the youth will need as an adult. By building a comprehensive multi-agency team, the team will ensure the student’s goals and profile are complete and they can provide a full array of transition services to address any gap in the students skills and experiences.
Once the team has identified those various multi-agency team members who could share valuable transition assessment data, complete the below team roster so everyone can be sure to include their information now and potentially return to them in the future for updates.
